ADHD (Attention Deficit Hyperactivity Disorder)

Attention Deficit Hyperactivity Disorder (ADHD) is a neurobiological condition that primarily manifests in childhood but often persists into adulthood. It is characterized by symptoms of inattention, hyperactivity, and impulsivity, which can significantly impact academic performance, social relationships, and daily life. 

About the Symptoms:

Inattention: People with ADHD often have difficulty concentrating on tasks, following instructions and organizing activities. They may lose objects needed to complete their tasks, forget commitments and make careless mistakes.

Hyperactivity: Manifests as a constant need to move, talk excessively and interrupt or be in constant motion. People may appear restless or unable to sit still in situations requiring calm or waiting turns.

Impulsivity: Involves acting without thinking about consequences, which can lead to interruptions in conversations or activities, and rash decisions.

How ADHD Affects Life:

– Academics: Difficulties concentrating, following instructions and maintaining organization, which may result in poor academic performance.

– Social Relationships: Difficult interactions due to impulsivity and inability to properly interpret social cues.

– Family Life: Tension in family dynamics, disorganization and communication between parents and children.

– In relationships: Communication problems or impulsivity that affect trust.

– At work: Forgetting meetings, unfinished projects and poor time management.

ADHD is classified into three presentations or subtypes according to DSM-5 (Diagnostic and Statistical Manual of Mental Disorders, 5th Edition):

– Predominantly inattentive: Difficulty concentrating, organizing or following instructions.

– Predominantly hyperactive/impulsive: Excessive restlessness, interruptions or impulsive actions.

– Combined: Symptoms of both presentations.

Note: Previously distinguished between ADD (without hyperactivity) and ADHD, but now considered a single spectrum.

Neurobiological Basis of ADHD

ADHD has no single cause. Genetic predisposition is considered to play an important role, as there is a high rate of cases in families. In addition, other factors potentially associated with its development have been identified:

  • Brain origin: Differences in structures related to impulse control, working memory and attention.
  • Neurotransmitters: Alterations in structure and activity of certain brain areas, especially those related to attention and impulse control, imbalances in dopamine and norepinephrine.
  • Genetics: Studies show that heredity is a key factor, with higher prevalence in first-degree relatives, heritability rate of 74-88%. Additionally, there are specific genetic syndromes – particularly Williams syndrome (deletion on chromosome 7) and DiGeorge syndrome (deletion 22q11.2) – that are frequently associated with clinical manifestations similar to ADHD due to shared alterations in neural circuits regulating attention and impulse control.
  • Environmental: Factors such as prenatal exposure to tobacco, alcohol or drugs and complications during childbirth (such as low birth weight or lack of oxygen) may increase risk of developing ADHD. Epigenetics also plays a very important role here.

– 9.8% of children (6-17 years) diagnosed with ADHD (CDC, 2023).

– 4.4% of adults (18+ years) live with ADHD (National Institute of Mental Health).

– 42% increase in childhood diagnoses (2003-2023).

Frequent Comorbidities:

– 77.9% have at least one other mental disorder:

– Anxiety (33%)

– Learning disorders (45%)

– Depression (17%)

– Conduct disorder (14%)

– Increase in adult diagnoses (+123% since 2007).

Alarming Data:

– 40% of children with ADHD develop substance use disorders in adolescence.

– 5 times higher suicide risk in untreated adults.

Diagnosis

ADHD diagnosis is made through a comprehensive evaluation that includes:

– Clinical Interviews: With child, parents and teachers to understand behavior in different settings.

– Standardized Questionnaires: To measure frequency and severity of symptoms.

– Psychological Assessments: Which may include attention, memory and executive function tests.

– Scales such as Vanderbilt or Conners.

– Rule out anxiety, depression or learning disorders.

Diagnosis must be made by mental health professionals experienced in this disorder, and it’s important to consider functional impact of symptoms on individual’s life.

Medications:

Psychostimulants: Drugs like methylphenidate and amphetamines are the most common and effective options. These medications help increase availability of neurotransmitters like dopamine and norepinephrine, improving attention and behavior regulation.

Non-Stimulants: Medications like atomoxetine, which may be useful for those who don’t respond well to stimulants or have abuse problems.

Behavioral Therapies:

Psychoeducation: Provide family with information about ADHD to understand condition, its implications and strategies to support children with ADHD.

Social Skills Training: To improve individual’s interactions and relationships.

Behavior Modification: Implementation of reward systems to encourage positive behaviors.

School Support: Collaboration between parents, educators and specialists to create adaptive learning environment that maximizes school performance and supports child’s needs.

Complementary Interventions:
